Consultation with a neurologist - parkinsonologist for Parkinson's disease

The complex diagnostics of a neurologist - parkinologist includes:

- Primary consultation of a neurologist - parkinologist
- The study of the history of the disease, complaints, the main clinical symptoms with the definition of the main algorithms for further research, confirming the diagnosis (idiopathic Parkinson's disease, various forms of parkinsonism with the difficulty of establishing a reliable clinical diagnosis, parkinsonism with the development of cognitive impairment) and the prognosis of the course of the disease
- Evaluation of functions with diagnostic tests: cranial nerves; motor-reflex system; sensitivity; coordination systems (static, locomotor functions); peripheral nervous system;
- measurement of blood pressure;
- Evaluation of the motor sphere according to the international scale for assessing parkinsonism;
- Evaluation of available survey results;
- Appointment of missing examination methods;
- Issuance of recommendations for treatment.

Types of treatment for
Parkinson's disease.

Treatment of Parkinson's disease is carried out in three main directions:
1. Drug treatment - the use of antiparkinsonian drugs.
2. Neurorehabilitation treatment (for more details, follow the link)
– restoration of lost functions and prevention of their reappearance.
3. Neurosurgical treatment (more details at the link) - implantation of neurostimulators in the brain, as well as cauterization of certain points of the brain, which very well suppress the main disabling symptoms of the disease.

Parkinson's disease is the second (after Alzheimer's) most common disease that seriously affects the brain. Mostly people over 65 years of age get sick, and the older, the more sick people become. But recently, cases of illness in people of a much younger age, about 50 years old, have become more frequent.
Parkinson's disease is a progressive disease of the nervous system, which is expressed in impaired movement. Symptoms begin slowly, imperceptibly, which makes it difficult to immediately make a correct diagnosis.

What is Parkinson's Disease?

"There are about 25,000 people in Kazakhstan who have Parkinson's disease. Many people have not yet been diagnosed with this disease. There are only a few neurologists - parkinologists for such a number of people. In this regard, improperly selected treatment is an urgent problem today. To completely cure Parkinson's disease is still no one succeeded, but in our clinic we exclude many symptoms of the disease in order to continue a quality lifestyle."

How many people suffer from Parkinson's disease in Kazakhstan and in the world?

Currently, there are about 5-6 million patients with Parkinson's disease in the world. Accurate data on the epidemiology of this disease in the Republic of Kazakhstan is not available. Therefore, based on the above calculations, it can be assumed that at present in our country there are about 27 thousand patients with Parkinson's disease, and every year they become 1200 more.

What happens in the brain in Parkinson's disease?

For various reasons, nerve cells die off in a part of the brain called the substantia nigra. It is there that dopamine is formed - a chemical that is very necessary and important in the process of transmitting nerve impulses from the brain to the limbs. Due to the lack of dopamine, movement disorders appear. Over time, there are more dead cells, and less dopamine, and therefore the disease progresses. The causes of cell death can be certain genetic mutations, which are very rare. As a rule, external toxic substances, which are very common in the environment, have a negative effect on the brain. These include chemicals, herbicides and pesticides, toxins, and the like. We noted that the disease most often begins after a sudden or chronically caused stress - a close relative died, an injury occurred, a major operation, or a person was under a strong burden of problems for a long time, slept poorly and was a lot of nervous. Also important is the lack of nutrients for the brain - vitamins, trace elements, proteins and fats. These factors lead to a sharp decrease in the protective properties of the brain, and toxins begin to actively penetrate into the cells and begin to destroy it. At the same time, pathological proteins can be deposited in the brain, which should not normally be present, which leads to the death of nerve cells.

Risk Factors for the Development of Parkinson's Disease

риск факторы болезни Паркинсона

1. Age. The older the age, the higher the risk of getting this disease.
2. Heredity. There is a high risk of getting sick in people if there are already people with such a disease in the family.
3. The gender of the person. Men get sick more often than women.
4. Toxins. Long-term interaction with herbicides and pesticides increases the risk of disease. That is why rural residents are more likely to get sick than urban ones.

Complications in Parkinson's disease.

осложнения Болезни Паркинсона

1. Memory disorders up to dementia, which can develop in the later stages of the disease.
2. Depression and emotional changes. Often, patients, even in the early stages of the disease, suffer from depression, decreased motivation, fear, and anxiety.
3. Sleep disorders in the form of sleep disturbance, superficial sleep, excessive movements in sleep. These are all the so-called non-motor symptoms of Parkinson's disease, which are often more pronounced than motor ones, and which are more difficult to treat and require a special approach.
4. Swallowing disorders due to excessive salivation, which can lead to salivation. There may also be chewing disorders and, because of this, nutritional problems.
5. Constipation experienced by most patients. The reason is a slowdown in bowel movement and digestion of food.
6. Disorders of urination can also manifest themselves in the later stages of the disease, manifested by urinary retention or incontinence.
7. Changes in blood pressure. It happens rarely, but takes a sudden form in the form of orthostatic hypotension.
8. Violation of smell, which, according to many experts, is an early symptom of the disease.
9. Chronic fatigue, the appearance of which cannot be explained by other reasons.
10. Pain, especially in the legs, which is associated with improper muscle tension or impaired innervation.
11. Sexual violations.

Diagnosis in Parkinson's Disease

Diagnosis of Parkinson's disease usually consists in a thorough clinical examination of the patient, identifying specific symptoms, identifying them as the disease progresses, and excluding other symptoms that are unusual for the disease. Unfortunately, there are fewer classical forms of the disease, and the clinical experience of a parkinsonologist, as well as data from additional examinations, is very important.
Usually, all patients undergo magnetic resonance imaging (MRI) of the brain, even several times over time. Parkinson's disease is not characterized by any gross changes on MRI. Doctors prescribe tomography to rule out other diseases. Of the laboratory research methods, special tests are prescribed that examine metabolic disorders that may be a risk factor for brain damage. However, there is still no specific analysis that could confirm or rule out Parkinson's disease. To diagnose the disease, a levodopa test is necessarily used - a test for the effect of medicinal levodopa on the symptoms of the disease. By conducting this test, the doctor receives very valuable information about the cause of the disease, as well as the possibility of its treatment.
In some cases, it takes time to accurately diagnose the treatment, so the doctor may prescribe additional appointments and monitoring. It is very important to record in writing all the problems that arise during the day in the form of a diary. This gives a great help to the doctor in determining the tactics of treatment.
